By submitting this enrollment form, I confirm that the information being submitted is accurate and complete, to the best of my knowledge. I also understand that because the Green Business Program does not work with all business types, and does not have standards for all business sectors, my business may not be eligible for certification. We commit to responding in a timely and complete manner to staff, auditors and/or inspectors contacting us to schedule a visit and to requests for information. We understand that the program may not be able to work with us if we are unresponsive to such requests within a reasonable period of time.
